Community Health Systems Wound Care Nurse / FT / 1st in Berwick, Pennsylvania
Assess the integumentary system of all residents at the Berwick Retirement Village. Plans and implements course of nursing care, as needed. Evaluate the effectiveness of each care plan.
Responsible for the delivery of patient care through the nursing process of assessment, diagnosis, planning, implementation, and evaluation. Coordinates and provides age-specific care for persons with bowel and urinary diversions, draining wounds, fistulas, pressure and vascular ulcers and incontinence. Educates staff, families and residents and the community about the specialized care of selected disorders of the gastrointestinal, genitourinary and integumentary systems. Collaborates with other professional disciplines to ensure effective and efficient resident care and the achievement of desired resident outcomes. Collects data for care improvement opportunities. Monitors wounds/open areas, completes reports, and reports to the BRV CQI meeting monthly
Requires an associate's degree and is certified as a registered nurse. Familiar with standard concepts, practices, and procedures within a particular field.
