About this team
The Behavioral Health Access Center is a state of the art clinical assessmentand patient placement hub. Composed ofRegistered Nurses (RN), Social Workers, Therapists, and Providers who workcollaboratively to provide behavioral health assessments and referral/placementservices for patients located throughout CHS hospitals and affiliated providerswho may benefit from inpatient and outpatient behavioral health services.
What you ll do
As a Clinical Assessment Specialist you will coordinate the transfer andacceptance of patients for all facilities in your region. You will collect relative clinicalinformation for a transfer request and work collaboratively to facilitate theassessment, disposition, and placement.
You will work with multiple departments within each hospital to fosterrelationships with referring facilities, physicians, other behavioral healthand hospital staff.
Here s a brief example of some of thework our Clinical Assessment Specialists do day-to-day;
● Ensure every patient interaction and assessment promotesthe well-being and safety of the behavioral health patient.
● Make psychiatricrisk assessments of behavioralhealth patients to assist making appropriate level of care decisions.
● Provide assistance with a patient's Discharge Safety Plan,in conjunction with generating the best patient clinical outcome. As well asformulating diagnostic impressions based upon review of systems, history,symptoms, and current presentation.

Requirements you need
Masters Degree in Social Work, Psychology,Counseling, or related field of study.
3-5 yearsof direct clinicalbehavioral healthcare experience.
Licensedin the state of Tennessee without restrictions (LCSW/LMSW, LPC/MHSP, , , withthe ability to obtain licensure in additional states as required.
