TheCore Measures Abstractor/Quality Coordinator (CMA/QC) is responsible for performing concurrent and retrospective quality and case reviews to assure that evidence based best practices are utilized. This is accomplished by using the Joint Commission and Center for Medicare and Medicaid Services Core Measure Guidelines to collect and enter data into database. This position requires ongoing consultation and education to medical and administrative staff with respect to the application of clinical core measures. The CMA provides support for core measures to hospital staff by identifying resource needs, explaining methodologies/tools to be used, identifying issues and problem resolution while providing analytical support and team leader prep; in an effort to alert personnel of issues and to assist in resolving the challenges/barriers that can affect the smooth execution/implementation of clinical core measurement activities. Therefore theCore Measure Abstractor assumes a leadership role and must be able to relate well with all medical staff and ancillary departments. While functioning as the Quality Coordinator the selected individual will be responsible for reviewing specific charts for data collection and analysis in an effort to support the facility wide "Performance Improvement Program" making recommendation for improvement.Responsible for clinical identification, risk evaluation and coordination of corrective action implementation related to risk issues. Provides intervention and education related to risk management issues to promote safe work practices and quality care and services; in an environment that is beneficial to the safety, health and well being of all patients, visitors and hospital staff. Coordinates risk programs with all hospital departments, administration and CHS/Hospital appointed attorneys. Reports real and potential risk situations to the Governing Board, medical staff, administration, hospital departments and committees, as appropriate. Responsible for establishing and monitoring methods to avoid, eliminate and/or reduce risk situations associated with the provision of patient care and services.
Certifications / Licenses:
- Current Louisiana Registered Nurse License
- BSN preferred
Core Measures abstraction and Quality Coordinator experience preferred
Previous acute care clinical experience required.
Attention to detail and follow-up necessary.
Must be emotionally mature and able to function effectively under stress.
Strong interpersonal skills, and the ability to communicate effectively with patients, families, hospital staff, physicians and community resources required.
Must have strong computer skills and the ablility to multi task.
Must be able to work independently.
