CHSPSC, LLC seeks an Registered Nurse (RN) Access Coordinator for its Franklin, TN, headquarters Access Transfer Center team.

About this team

The Access Center is a state of the art clinical assessment and patientplacement hub. Composed of RegisteredNurses (RN), Social Workers, Therapists, and Providers who work collaborativelyto provide behavioral health assessments and referral/placement services forpatients located throughout CHS hospitals and affiliated providers who maybenefit from inpatient and outpatient health services.



Your Job

What you ll do

As a Registered Nurse - Access Coordinator you will coordinate the acceptanceof patients in the Community Health Systems (CHS) system. Using approvedclinical criteria and approved guidelines for policies and procedures, you willperform initial admission screening and work in collaboration with multipledepartments within the hospitals and grow relationships with nursing, physicianand facility staff. You will work in conjunction with our Capacity Coordinatorto problem solve, identify trends, and report barriers to patient throughput.

Here s a brief example of some of thework our Registered Nurse – Access Coordinators do day-to-day;

--{PS..4}● --{PS..5}Manages the admission and transfer of patients,while providing a timely and considerate duty of care to thosepatients. Providecomprehensive follow up to the referring facilities.

--{PS..6}● --{PS..7}Acts as a liaisonacross many points of communication within Community Health Systems affiliatesfrom physicians to nurses, as well as departments and facilities. Also withpatient and family to communicate plans and processes during patient flow.

  • Process and documentpatient information, with respect to all guidelines and policies in placeincluding regulatory knowledge ofEMTALA,CHS Ethics and Compliance policies, and quality initiatives.

Requirements you need

Graduate of anaccredited school of Nursing with three (3) years of related clinicalexperience in an acute care setting (ED or Critical Care Preferred).

Current licensureState of Tennessee (RN) and proficient in EMTALA.
