Community Health Systems Blended Case Manager - Case Management - Community Counseling - 1.0 FT D in Wilkes Barre, Pennsylvania
Blended Case Managers provide assistance to persons with serious and persistent mental illness in a variety of ways to gain access to needed resources such as medical, social, educational and other services through natural supports, generic community resources and specialized mental health treatment, rehabilitation and support services. This service is expected to achieve specific outcomes of independence living, vocational / educational participation, adequate social supports and reduced hospitalization.
Bachelor s degree with major course work in sociology, social welfare, psychology, gerontology or other related social sciences.
Blended Case Managers who arenot mental health professionals shall be supervised by a mental healthprofessional and have one of the following:
1) A Bachelor s degree with major course work insociology, social work, psychology, gerontology, anthropology, politicalscience, history, criminal justice, theology, nursing, counseling, or education.
2) A registered nurse.
3) A high school diploma and 12 semester credit hours insociology, social welfare, psychology, gerontology or other social science and2 years experience in public or private human services with 1 year in directclient contact.
4) A high school diploma and 6 years of mental healthdirect care experience in public or private human services with employment as anintensive case management person prior to April 1, 1989
Valid Pennsylvania Drivers License must be maintained throughout entire tenure in this position.
