Community Health Systems Case Manager (Weekend PRN) in Cedar Park, Texas


Located minutes from downtown Austin, Cedar Park Regional Medical Center is a level IV Designated Trauma Center with more than 600 healthcare professionals working together to provide inpatient, outpatient, surgical and emergency care for Cedar Park and the greater Austin region.

Position Overview

We are currently seeking an experienced Registered Nurse Case Manager to join our team! This is a PRN position working 1-2 weekend shifts per week, or as needed.

The Case Manager conducts systematic data collection and review to identify the appropriate of level of care and length of stay for services rendered and facilitates meeting patient needs in a way that improves the quality, efficiency and affordability of health care.


  • In conjunction with other members of the interdisciplinary care team, coordinates effective and efficient patient care during the hospital stay from admission to discharge

  • Collects and uses information from medical record reviews to compare results of outcomes to evaluate effectiveness of Case Management Program and improve patient care

  • Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively

  • Screens patient for discharge planning risks and/or needs to make appropriate referrals to the next level of care

  • Assessment skills to interpret the significance of circumstances related to the patient s medical problem and potential discharge concerns

  • Contributes to the care planning process during entire hospital stay and communicates goals to expedite discharge

  • Assist with planning continuum of care with physicians and coordinating required interventions for patients transferred to other facilities

  • Utilization review of patient charts to ensure appropriate utilization of hospital resources and facilities

  • Compliant with EMTALA guidelines and hospital requirements for MOT completion and log entries

  • Seeks to improve hospital performance on Quality Indicators

  • Follow-up visits and phone calls with patients and/or families as needed


  • Minimum of an Associate s Degree inNursing (ADN) from an accredited RN program. Bachelor s Degree in Nursing (BSN)preferred

  • Current license to practice as aRegistered Nurse in Texas, or license with a compact state

  • American Heart Association (AHA)approved Basic Life Support (BLS) certification required upon hire

  • Minimum of 2 years of acute care CaseManagement experience, preferably in ICU or ED

  • Recent with needs assessment,utilization review and discharge planning

  • Solid understanding of reimbursementstructures

  • Understanding of CMS regulations inthe acute care setting

  • Ability to read and comprehend complexclinical and financial data

  • CCM highly desired
