Position Overview

We are currently seeking an experienced Licensed Medical Social Worker (MSW) to join our team! This is a PRN position working 1-2 weekend shifts per week (4-8 hours per shift). The MSW managed domestic violence and sexual assault patients and places psychiatric patients. This includes a wide variety of high level patient care activities to include admission assessment, screen for appropriateness, protocol development and monitoring development of plan to address physical and psychosocial needs.

Responsibilities

  • Interviews clients and their families and coordinates and plans programs and activities to meet their social and emotional needs

  • Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle

  • Coordinate the plan of care among all members of the health care team

  • Coordinate services and discharge planning, patient/family teaching, and monitoring of cost effective health case across the continuum of care

  • Completion of all initial assessments within 24 hours of admission

  • Successfully meet resource management expectations, assist to reduce practice variations and enhance patient outcomes

Qualifications

  • Master s Degree in Social Work required

  • Must be a Licensed Social Worker in the State of Texas

  • Minimum of two (2) years of Social Work experience in an acute care hospital setting

  • Acute care discharge planning experience required

  • Must be available for days, evening, weekend and/or holidays

  • Ability to work independently with sound judgment and a caring, professional demeanor

  • Must possess strong written and verbal communication skills with the ability to resolve high stress and "crisis" situations in a calm, respectful manner
