Community Health Systems Patient Navigator/Social Worker (Case Management) - FT in Corsicana, Texas


The Social Worker/Patient Navigator is accountable for the organization, sequence of services, and resources that are necessary and appropriate for the achievement of patient outcomes within effective time frames on a specific group of patients. The goal is to create a positive patient experience through enhanced efficiency and convenience for unassigned patients presenting to hospital Emergency Department or as a Direct Admission. The Social Worker/Patient Navigator will facilitate appointments for unassigned patients to include assistance with selection of a PCP, scheduling provider appointments on the patient s behalf, phone call to the patient the day before appointment, and follow-up call to the patient the day following appointment. Inpatients wishing assistance with establishing a PCP will also be offered Patient Navigator Services. In addition, the Social Worker/Patient Navigator will coordinate the plan of care among all members of the health care team. The Social Worker/Patient Navigator must have ability to practice under minimal supervision. The Social Worker will perform a wide variety of high level patient care activities to include admission assessment, screen for appropriateness, protocol development and monitoring development of plan to address physical and psychosocial needs. Coordinate services and discharge planning, patient/family teaching, and monitoring of cost effective health care across the continuum of care. The Social Worker/Patient Navigator must meet resource management expectations, assist to reduce practice variations and enhance patient outcomes.



Masters in Social Work


Currently licensed to practice as Social Worker by the Texas State of Social Worker examiners.
