Community Health Systems Nurse Practitioner/Physician Assistant - Surgery/Bariatric in Fort Wayne, Indiana

Description:

It is the intent of the Board of Directors that the Nurse Practitioner/Physician Assistant develop their own practice of medicine and also complement the health care services of their physician colleagues in the group. Nurse Practitioners/Physician Assistants are to provide the highest quality health services in keeping with the group's mission. Nurse Practitioner/Physician Assistant is responsible for assuring the delivery of effective individualized care in our service. Assesses, develops, and implements plan of care for hospitalized patient, which includes identification of actual and potential problems, hospital discharge, and follow-up care. Evaluates patient response in relation to established goals and treatments.

Mon-Fri with some weekend and evening coverage

Qualifications

Education:

  • Graduate of an accredited Nurse Practitioner/Physician Assistant program. If credentialed under other guidelines, the NP/PA must be able to attain certification and re-certification.

Certfications/Licenses:

  • Must possess a current State of Indiana Nursing registration if NP.

  • Must have current certification as a Nurse Practitioner or Physician Assistant or be registered to take the next-offered certification examination. Individuals awaiting the results of the certification exam may be hired, pending successful completion of the exam.

Experience:

  • Minimum three years of general clinical experience.

  • ICU/ER experience preferred but not required.
