Community Health Systems Registered Nurse- Breast Navigator (Full-time, 32 hours per wk.) in Fort Wayne, Indiana

Description:

Under the direct supervision of the IR/CBH Team Specialist, individual will serve as the key navigator for direction of patient flow and care of all intervention procedures in the Center for Breast Health. Coordinates procedure results with patient according to referring physician preferences and provide additional supportive services as requested. Providing input in MRS data collection for accurate medical documentation. Maintains good working relationships with radiologist and surgeons involved with patient care.

Qualifications

Minimal Acceptable Job Qualifications

Licensure: Current Indiana State License RN

· BCLS within 30 days of hire

· ACLS within 90 days of hire

· Breast Navigator Certification required within 2 years of hire

Experience: Preferred mammography experience or equivalent preferred.

Education: Completion diploma or degree program in nursing field with additional education experiences preferred.

Other:

Participates in related committees, as required.

Participatesin Breast Tumor Board.

Collaborates with other hospital team members, physicians, physicians staff, patients, families, and other agencies regarding integration issues and reporting on integrated clinical information.

Prepares reports and other related documentation, as required.

Demonstrates strong working knowledge of nursing practices and system integration.

Requires strong analytical and organizational skills in order to analyze, develop and interface requirements in coordination with customer needs and others.

Demonstrates strong interpersonal and communication skills, both verbal and written, in order to interact effectively with internal and external customers as needed.
