Community Health Systems Assessment and Referral Coordinator - Inpatient - First Hospital - 0.2 PD D/E/N/W in Kingston, Pennsylvania


Under the administrative supervision of the Chief Nursing Officer and the clinical supervision of the Medical Director, the Assessment and Referral Coordinator accepts calls for requests for clinical services and attempts to satisfy the needs of the client by assessment, admission or referral.


The Assessment and Referral Coordinator will have a degree in Psychology, Social Work, Sociology, Criminal Justice, Counseling or a related field or a High School Diploma and 12 semester hours in Sociology, Psychology, Social Work, Criminal Justice, Counseling or other Social Science with 2 years of mental health direct care experience or a High school diploma with 3 years of mental health direct care experience.

Must be computer literate; data entry proficiency. Demonstrates verbal and written communication skills and the ability to express ideas in a clear, concise and legible form; ability to make independent decisions, when necessary; ability to interface with all disciplines; and exercise tact, diplomatic and good judgment at all times.
