Community Health Systems Patient Care Coord in Starke, Florida


SUMMARY: As an essential member of the nursing leadership team, provides clinical oversight, supervision, and optimization of patient care, patient flow, and staffing, by serving as the nursing leader to the Medical-Surgical unit, Emergency Department, and as appropriate, Perioperative Services, during “after hours.


  1. Serves as administrative resource and the nursing management designee during off-hours, with delegated authority over all caregivers on the hospital patient care units. Collaborates with CNO and/or department director(s) as appropriate.

  2. Provides leadership, coaching, and performance management to caregivers, to support optimal patient care, outcomes, and patient experience. Serves as role model, reinforces expected behaviors and processes to assure consistency, provides coaching and redirection as indicated.

  3. Rounds frequently in all patient care units to support timely, appropriate patient placement, and serves as a clinical resource as needed, particularly during surge in patient volumes and/or increased acuity. May provide direct care on an intermittent or short-term basis.

  4. Provides clinical support and coaching with changes in patient condition. Facilitates escalation via chain of command as indicated.

  5. Supports business of patient care units, including daily time and attendance adjustments, monthly schedules, transfer logs/reports, and staffing adjustments as needed to meet patient care needs and productivity standards. Assures caregivers are receiving meal and rest breaks as appropriate.

  6. Monitors progress with orientation of, competency of, unit education to, and ALC compliance/completion by assigned caregivers.

  7. Performs structured rounding on assigned caregivers and on patients to enhance caregiver engagement and patient experience. Documents and communicates results to CNO and appropriate director(s) for follow-up as indicated.

  8. Monitors continuous compliance with regulatory standards and performance of high risk procedures, such as moderate sedation and blood transfusion, to assure consistency with defined processes and to provide coaching/education if indicated.


  1. Graduate of an accredited school of nursing

  2. Current, active, unencumbered Florida license as Registered Nurse

  3. Minimum five years clinical experience in Medical-Surgical and/or Emergency Department

  4. Minimum two years clinical supervisory or management experience required

  5. BSN highly preferred

  6. BLS, ACLS, PALS certifications current or obtained within 3 months of hire (BLS within two weeks of hire)
