Community Health Systems Social Worker - Trauma Services - 0.2 PD (D/R) in Wilkes Barre, Pennsylvania


The Social Worker is responsible for assessing, counseling, formulating, and coordinating discharge planning services for patients. The Social Worker provides direct and indirect social work services to patients and families to facilitate discharge planning and to enhance their social functioning, particularly as it relates to the consequences of illness and disability. Intensive counseling is provided when indicated. The Social Worker functions under the direct supervision of the Director of Case Management. Serves as an interdisciplinary team member to coordinate the delivery of health care services leading to quality outcomes for the inpatient, ambulatory surgery and observation population with the limits of the financial and social resources available in a manner reflective of the Hospital s philosophy


Bachelor's Degree in Social Work from accredited school of social work required. Masters preferred.

Medical casework experience desirable; community service experience beneficial. Understanding of Case Management principles; knowledge of community agencies and service organizations; casework skills in individual, group and family modalities; crisis intervention; communication skills; effective program presentation; discharge planning; interpret medical and psychiatric data; interpersonal skills; capability of problem solving theories and techniques.

Licensed as a Social Worker in the State of Pennsylvania.
